Get Help Planning Goals for Your Life with a Social Worker

The Linkages Social Worker works with the you to create a plan based on the goals you identify to improve your life situation. The Linkages Social Worker helps to create action steps and track those goals during a 6 month to 1-year period. The Linkages Social Worker will stay in contact by telephone and will make in-home visits.

How to Apply

You or your caregiver can complete and submit the online application or call 707-565-4636.

We accept voluntary contributions for services we provide to the community through the Area Agency on Aging. There is no obligation to contribute; it's purely voluntary. No eligible individual shall be denied service because of failure to contribute. Contributions will be private and confidential to respect the individual's contribution or lack of contribution. If at any time you would like to contribute to this program. please make a check payable to the County of Sonoma Are Agency on Aging, reference "AAA volunatry contribution" and select which program service you would like to contribute to.
