The County of Sonoma's Employee Assistance Program (EAP), provided by Claremont, offers confidential support services to employees and their eligible family and household members.

Accessing EAP Services Through Claremont

You can access EAP services and resources through Claremont in one of two ways:

Option 1: Contact a Member Care Navigator

Call (833) 319-5343 to connect with a Member Care Navigator, who can help you locate resources, coordinate services, and connect you with a provider.

Option 2: Search for a Provider Online

  1. Register and activate your account by contacting Claremont at (833) 319-5343
  2. Visit their website at claremonteap.personaladvantage.com
  3. Log in using your username and password
  4. Select "Provider Lookup" to search for providers within the network

Important: If you choose Option 2 and find a provider through the online Provider Lookup tool, please contact Claremont to obtain a pre-authorization before scheduling your appointment. This helps ensure your services are properly authorized and covered through the EAP.

Your Member Care Navigator can help make accessing care and support easier by providing guidance, coordination, and practical next steps. They can:

  • Share resources tailored to your unique situation
  • Assist with family, caregiving, and work-life concerns
  • Connect you with additional benefits and support services
  • Provide ongoing follow-up and continuity so nothing falls through the cracks

Eligibility for EAP Services

All County of Sonoma employees—including full-time, part-time, and extra-help employees—are eligible for Employee Assistance Program (EAP) services. These services are also available to eligible family and household members at no cost.

Eligible family and household members include individuals living with the employee, such as a spouse, domestic partner, child, grandchild, parent, grandparent, or other household member. In addition, dependent children under age 26 who live away from home remain eligible for EAP services.

Confidential Counseling Services

Talk one-on-one with an experienced, licensed counselor for support with stress management, relationship concerns, work-life balance, grief and loss, and other personal challenges. Counseling services are available face-to-face, online, or by phone—whichever is most convenient for you.

Claremont EAP provides employees and their eligible family and household members with confidential, short-term counseling services at no cost. As with all EAP services, your conversations remain strictly confidential.

Counselors can provide support for a variety of personal and life challenges, including:

  • Marital and relationship concerns
  • Anxiety
  • Domestic violence concerns
  • Substance use and recovery support
  • Depression
  • Grief and loss
  • Stress and trauma
  • Parent-child conflicts

Legal Consultation Services

One 30-minute legal consultations per each separate legal matter at no cost, 25% reduction from the normal hourly rate if member retains attorney or mediator.

Claremont's financial consultants provide objective, confidential guidance. They do not sell financial products or services—their sole focus is helping you make informed decisions and achieve your financial goals.
