The County of Sonoma's Employee Assistance Program (EAP), provided by Claremont, offers confidential support services to employees and their eligible family and household members.

When life becomes challenging, EAP professionals can provide information, guidance, and support to help you and your family navigate personal and professional concerns, manage everyday stress, and build healthy, fulfilling relationships.
EAP benefits include:
- Six (6) face-to-face, text, phone, or virtual counseling visits per situation
- Legal consultation services
- Financial consultation services
- Work/life referral services
- Online resources and tools

Accessing EAP Services Through Claremont
You can access EAP services and resources through Claremont in one of two ways:
Option 1: Contact a Member Care Navigator
Call (833) 319-5343 to connect with a Member Care Navigator, who can help you locate resources, coordinate services, and connect you with a provider.
Option 2: Search for a Provider Online
- Register and activate your account by contacting Claremont at (833) 319-5343
- Visit their website at claremonteap.personaladvantage.com
- Log in using your username and password
- Select "Provider Lookup" to search for providers within the network
Important: If you choose Option 2 and find a provider through the online Provider Lookup tool, please contact Claremont to obtain a pre-authorization before scheduling your appointment. This helps ensure your services are properly authorized and covered through the EAP.
