Disease Reporting
The state of California requires doctors, surgeons, nurses, practitioners, coroners, dentists, veterinarians, administrators of health facilities and clinics to report cases or suspected cases of certain diseases and conditions to the local health department. Anyone in charge of public or private schools or preschools also must report cases or suspected cases of these diseases.

How to Report
The Confidential Morbidity Report (CMR) forms listed below are used to report most reportable diseases and conditions.
To report tuberculosis (TB): See Tuberculosis Control for more information
To report animal bite and exposures: Complete Animal Services Bite reporting form or call us directly for public health consultation on wildlife encounters and rabies prophylaxis recommendations and information.
To report HIV/AIDs: See HIV/AIDS Reporting for more information
To report all other reportable conditions: please complete the Confidential Morbidity Report (CMR): CDPH 110A
To report conditions which may impair the ability to operate a motor vehicle, such as lapses of consciousness or control, Alzheimer's disease: CMR CDPH 110C
Note: Please call, in addition to submitting a CMR, for all high-risk diseases and conditions (as outlined in Title 17 Reportable Diseases and Conditions)
