• Use this form to report injuries and/or property damage sustained by a third party (non-employee). If your department has its own incident report form, you may use that instead of this form.

Property Damage Incident Report (PDF: 117 kB)

• Use this form to report damage to County property.

Vehicle Incident Report Form

• Use this form to report vehicle accidents or incidents involving a vehicle.

 Send completed reports to Risk Management at liability@sonomacounty.gov or fax to 707-565-6192. Keep a copy for your records.
