The County of Sonoma's Workers' Compensation program compensates employees for disability leave and medical treatment as a result of a work-related injury or illness. Workers’ Compensation benefit regulations are established by the State of California’s Division of Workers’ Compensation.

The Disability Management (DM) Unit is available to assist you with your work-related injury or illness questions, see Staff Assignments for contact information.

How to Report a Work Related Injury or Illness:

Employees:

  • An employee should report a work related injury or illness immediately to their supervisor or acting supervisor.

Medical Care: If you need medical care, please contact one of the following:

  • Kaiser Occupational Health Santa Rosa 707-566-5555
    • 3975 Old Redwood Hwy. Medical Office Building 5, Suite 152, Santa Rosa
  • Kaiser Occupational Health Petaluma 707-765-3800
    • 3900 Lakeville Hwy. Medical Office Building 2, Petaluma

    • 1221 N. Dutton Ave., Santa Rosa
  • Concentra Rohnert Park 707-586-4320

If you need urgent care, please go directly to an emergency room and notify your department. You are not required to complete a claim form prior to seeking treatment; however, you must inform your supervisor or department's human resources staff that you are seeking treatment.

If you seek treatment, the medical office will release you with a work status report. Please provide a copy to your supervisor or HR contact. We don’t require any diagnosis information; simply include your work status, indicating whether you are able to return to full duty, have work restrictions, or have been placed off work.

Supervisor or Acting Supervisor:

  • Supervisor or acting supervisor will need to complete the County of Sonoma’s incident reporting within 24 hours of notice.
  • If the injury is serious, please verify whether it meets the Title 8, Section 330 definition for reporting to Cal-OSHA within 8 hours of notification by contacting HR Safety Unit at (707) 565-3208.

If access to the online incident reporting is not available, complete the supervisor report and email completed Supervisors Report to: workcomp@sonomacounty.gov or fax to (707) 565-3501.

What to Expect:

  • The supervisor report of injury is referred to the claims administrator.
  • Employee and supervisor or acting supervisor may receive a call or email from the claims administrator requesting critical information.
  • Please return the call or email, as failure to respond could result in delay or denial of the claim.
